- Online Digital Evaluation & Management services refer to medical care provided through secure patient portal messages or electronic communications regarding the evaluation or management of a medical condition.
- These services are intended for clinical assessment and medical decision-making—not for scheduling, billing questions, prescription refill requests without evaluation, or other administrative matters.
- Charges may apply when portal communications require provider evaluation and medical decision-making. Fees are based on the cumulative time spent by the provider over a seven-day period.
- If your medical concern requires an in-person visit, telehealth appointment, or emergency care, you will be advised accordingly.

Current Medical Cannabis Evaluation Prices - Good Thru 12/31/2026
- Medical cannabis evaluations are conducted in accordance with Florida law and applicable regulations.
- Payment covers the medical evaluation only and does not guarantee certification or approval for medical cannabis.
- Certification is based solely on the provider's independent medical judgment and whether the patient meets Florida's legal eligibility requirements.
- State registration fees, dispensary products, and any applicable renewal fees are separate and are not included in the evaluation fee.
- Telehealth appointments for recertification are available only when permitted under current Florida law and when clinically appropriate.
